If you want to use the cellular connectivity on the watch (LTE), you'll have to pay $10/mo. Otherwise if you don't want to pay for cellular, you can use it as a normal Apple Watch

No monthly fee unless you buy wireless watch which lets you connect to your phone plan. Once you actually call and connect to your phone plan then you’ll have a charge. Other than that you can buy whichever watch out right
