Is this a modem for internet with a phone jack? null

I don’t believe so. I have it plugged into a phone jack and then have a phone and a fax machine inputted into to to split that phone line.

no, not a modem. simply turns a single phone jack into a three way so you can add your land line, a fax line and one other phone line if needed.
