• Yes. Also has adapter for TVs with the connectors in the middle.

• Yes it did come with mounting to the tv screws but my tv required a larger screw, Had to go to Lowes to get the correct screws.

• Yes, there are 2 sets of screws supplied. The longer set of screws are in case you need to use the spacers (also included).
