Mine says device 1 connected, then I make sure the bluetooth button is on during the call. Other than that, just start playing music and it should work. I have the note 4 also. I did the check for updates and installed the latest OS before using the sliver. Settings->general->about->software updates->check for updates. Mine works on version 5.0.1

Sorry, there was a problem. Please try again later.Return it and replace it; you just got a bad unit. I too have the Galaxy Note 4 and have had absolutely no issues whatsoever. Also, I just purchased the Note 4 last week so I had the blue tooth headset with no issues with my prior phone (Note 2). Good luck!
