“Can you just run a cable from the router to the device with ear instead or do you have to run it to the one without and the plug the one with ears somewhere else. Or can you run a cable to the one without the antennas and then run a cable to the other one”

The one with ears is the transmitter, it is not meant to be connected to the router. You should run a cable to the one without antennae, plug it into the wall, then plug the one with the antennae into wherever you'd like better connectivity.
