“How does one install them into areas of a ceiling that doesn't already have a light socket as the picture represents with the guy and his dog? Iam guessing hiring an electrician to go into the attic and connect wires and junction boxes?:)”

You will have to get a retro fit cut-in can at your local Menards or Home Depot

You don't have to use the screw in adapter, they can be wired. The adaptor is included cause the majority of retro fitting these it works be used.

Correct, you would need to install can light fixtures in your ceiling.
