“Simultaneous headphones and speakers? Is there a way to hook up headphones so that you can just mute the sound on the TV and use the headphones themselves to adjust the volume? I don't want to disconnect them daily, I have ruined TV's pulling the cord out every mornig and putting it back in at night, and I don't want tohave to go through a series of menus to switch the audio output.”

On my Samsung, it automatic. Just plug the headphone jack into the input jack in the back of the TV. The speaker will be killed. On my TV it's the input below the green audio "in" input. If you look closely, the headphone input jack actually has a tiny headphone logo next to it.
