• The campaign does not, but multiplayer does.

• From what I found, it does require internet connection to keep track of any unlocks you earn in the game from completing each mission, but it shouldn't need it to play the story mode. Obviously to play online, you will need a connection and either Playstation Plus or Xbox Live online service. Hope this helps and good luck.
