yes they should, mine came will everything except for installation cd which really isn't needed if you connect headphone to the xbox to update.

Yes, my refurbished set did come with a headset adapter.

It does come with a 1/8 in. (3.5mm) connector. An adapter may be need if you want to use in a different size audio port .

No but you do not need the adapter at all you just hook it up then you could talk in game and party you don't need it to be plugged in to your paddle at all
