“Why does headphone icon continue to show up randomly? Essentially when it comes up I can’t hear audio through actual TV. I don’t have anything plugged into headphone jack”

Are you sitting on the remote? If you aren't "butt surfing" with the remote try unplugging everything (this causes the TV software to re-boot).
