There are nozzles that are hidden until activate. Once activate the water pressure allows the nozzles to extend and spray the desired areas.

When no water is being directed to them, they are both hidden under the back of the toilet seat, near the water tank part of your toilet. When you use the little silver knob to direct water to either one of them (ones for rear ends and the other is for cleaning female body parts), and the pressure causes the nozzle to extend from its resting position and move forward, and then it starts spraying. When you put the control back to off, the water pressure goes down, and the nozzle retreats back to its hidden position. At no point are you defecating onto either nozzle.
