The small is generally for women and the large for men-it refers to the size of the wrist (about 5 1/2 -7 inches for the small and up to 8 inches or so for the large)

The only difference is the length of the band. I am a 5'8" female and I do not have a petite wrist. The small fits me with over half the adjustments holes left. Plenty big.
