The band that holds the device is easily adjusted; there are still 4 adjustment points left that I could tighten it further on my wrist. So, in terms of overall size, it should be ok for your wrist.
That said, what I found cumbersome is that the device itself (not the band) is a fairly broad and rigid size. Because my wrist is small, there are gaps on either side of that rigid structure where it doesn't meet skin. It's a little annoying, but it doesn't adversely affect how the device works for me. The green heart-rate monitors are in the middle of the device against my wrist, so they still find my heart rate.
A:AnswerNo you don't have to use a smart phone. You can sync it with a pc. You loose the nice notifications without a smart phone but you still have the other features. I run with mine and don't take my phone with me.

A:AnswerMy tracker does not count the flights of stairs accurately most of the time. I can see on the computer that I may climb 8 flights of stairs in a day, but I go down 18 flights on the same day. This is particularly true about the stairs in my home. During a recent trip, it counted the flights in my hotel accurately. It also records the flights at the gym well. The heart rate monitor is pretty consistently showing the same reading as the gym equipment. I think it misses some of the steps I take. The accuracy seems to be related to the tracker's position on my wrist. I have found an area where I get consistent readings. The tightness of the band also influences accuracy. The tracker may not be perfectly accurate, but it gives me valuable input about my activities so that I can track my fitness regime appropriately.
Initially, I noticed a bumpy rash on my arm just under the optical sensor shortly after swimming in a chlorinated pool. I was able to switch arms for a day or so, and am careful now to rinse the device thoroughly after exposure to the pool water, and have had no further problems. My health care provider believes this is my skin's response to the optical sensor's exposure to the chemicals from the water. He also cautioned to keep the tracker and my arm clean following exposure to perspiration.
