“Is the same as the Mohu Leaf "Glide"?? On Mohu's website, they have one called "Glide" which seems like the same as this one. There is no antenna on Mohu's website called "Supreme". If they are the same, why does Best Buy/Mohu call this one "Supreme"?”

The Leaf Glide is Mohu's version of the antenna and the Leaf Supreme is the Best Buy exclusive version of the antenna. Many retailers request that a "special" name be given to products they offer so they are different from other offerings.
