Q: once set up, can you use the tv or cable remote to adjust volume?
A: You have to use the remote provided

Q: TV is wall mounted so if I can't access the optical connector how do I connect the soundbar?
A: from my experience , I believe you would need to dismount the TV from the wall, plug in the optical connector and then remount the TV on the wall. The sound bar could be wall mounted below the TV.
